Is Root Canal Treatment Painful? The Truth, Step by Step

The words "root canal" are enough to scare many people. In modern dentistry, with good anaesthesia and precise instruments, root canal treatment feels much like an ordinary filling for most patients — and its purpose is to remove pain, not cause it.

What is the nerve of a tooth?

Inside each tooth is the pulp, living tissue with nerves and blood vessels running through narrow canals to the root tips. When deep decay, a crack or an injury lets bacteria in, the pulp becomes inflamed or dies, and an abscess may form. Inflammation inside a closed, hard space is what causes severe throbbing pain.

Signs you may need one

  • Severe or throbbing pain, especially at night.
  • Sensitivity to hot or cold that lasts minutes.
  • Pain on biting.
  • Gum swelling or a "pimple" near the tooth.
  • The tooth turning grey or dark.

Step by step

  1. Examination and X-ray to confirm the diagnosis and see the roots.
  2. Local anaesthesia, starting with a numbing gel; we only begin once the tooth is completely numb.
  3. Isolation with a thin rubber dam to keep the tooth clean and dry.
  4. Cleaning and shaping the canals and disinfecting them.
  5. Sealing the canals tightly.
  6. Restoration with a filling and usually a crown.

After treatment

Mild soreness on biting for a few days is normal and usually eased with ordinary painkillers. Do not eat until the numbness wears off, avoid heavy chewing on the tooth until the final restoration, and keep your crown appointment — a treated back tooth is more prone to fracture, so a crown protects it.

Root canal or extraction?

Extraction may seem quicker, but losing the tooth leads to shifting teeth, bone loss and the need for a bridge or implant later. Keeping your natural tooth is usually the better choice.
